If the doctor DC'd the patient then it is a break in need. If the reason for the break in billing cannot be documented then it is a break in service. Be sure to add narrative on the claim stating which scenario this is. Also it is helpful for break in need to describe the reason for the break in need. I have a fullproof one, no longer hypoxic. No matter the reason it all boils down to the patient being no longer hypoxic.
