What's your denial code? It could be a number of things, missing modifier,
missing narrative, etc..

It is being front end rejected when you submit the claim or is it being
denied after submission? What modifiers are you using when submitting the
cushion? What code is the w/c being billed with? Did you include a
narrative stating what w/c the cushion is being used with and that the w/c
is a rental?

I'm pretty sure that it needs a KX modifier, plus you can't have NU and RR on the same claim.
Also, I have been noticing that, in region A anyway, they are asking for extra narrative for equipment used on equipment that is still renting, like your wheelchair. For example, I recently had to add extra narrative for A7003, nebulizer cup & tubing, stating that pt is renting E0570. Anything to avoid paying a claim in a timely manner.
Hope this helps!

E2601 Wheelchair Cushion is only billed as a purchase. You would not rent and sell a cushion. Remove the RR Modifer.

One More thing I always put a full description of the cushion and size in the notes to avoid any other rejections. Example
Sunrise Medical- Item Number
Jay Basic Cushion
Seat Size 18 x 16

Did you provide the make model and serial number of the wheelchair that was provided 3 months ago?
My rule for Medicare is arm them with every piece of information you have to get the claim paid the first time.
Example:
Sunrise Medical Jay Basic Cushion Item Number SB16
Provided 2-19-2010
Medicare Rental Wheelchair provided 11-2009
Invacare Tracer SX5 Serial Number 123456
Seat Size 18 x 16

Good Luck!
