Default Commercial Insurance Question

I need some help from the Billing Guru folks. We are setting up a new software billing system and I need to confirm something on Modifiers. Do you need with Humana, Anthem, etc... any other modifiers other than RR and NU? Obviously you need the laundry list for Medicare but what about the others?

Default Commercial Insurance Question

I can't tell you which insurers they were, but I have called on denied
rental claims and was told they denied because the KH/KI/KJ modifier was not
present - we submitted w/ only the RR... I don't know about the KX mod but
probably couldn't hurt to include that too when applicable??

Default Commercial Insurance Question

We only use RR and NU on most items for commercial insurances. However, if the Anthem, Humana, etc. is a Medicare Advantage Plan, then of course, use the same modifiers as if you were billing Medicare.
