How do you all go about notifying the physician that the pateint must be re-evaluated for recertification of home o2? Do you send a form for completion to the doctor that has all of the coverage criteria that is needed for the patient chart etc?

We send patient notices approximately 60 days prior to their recert date; our cover letter has a place for the doctor to indicate the last follow up with the patient; this gets put into patient's medical record. The follow up to insure patient is seen is key.

We basically do the same the physician is sent a notification.

We send our recerts for group 1 patients a letter asking them to make a doctors appointment during specific days (to qualify with the 90 days prior per Medicare)

If it is a group 2 patient where new testing is concerned we send the notice to the doctor indicated the patient must be SEEN and RE-TESTED prior to the appropriate date. We send a copy of this letter to the patient as well.
