Default Medicare O2 pts eligible for new equip

Can I ask how everyone is handling replacing equipment for existing patients? Do you do it when you switch the equipment out due to problems? Do you have the patient sign something?

We currently monitor the number of months for our patients - as they turn to month 61 we have a technician go out to the house and discuss the options:

1. Continue on as is paying privately
2. Changing companies
3. Returning the equipment and discontinuing therapy
4. Starting a new cap rental period with our company

We have a letter they sign confirming what they choose to do and follow up accordingly.

We are doing the same thing with our customers, however I have had to send all my claims for new equipment to review in order to get medicare to pay. Are you having to do the same? What am I doing wrong. I enter all the URL info in the form. Any advice would be helpful. Thank you

Are you adding that this was an RUL in the narrative along with the original CMN date? Using "RA" modifier? It is also helpful to check the IVR same and similar to be sure the initial CMN date matches your records. The companies I have helped are doing these exchanges quite successfully and are now doing recert RUL CMNs!

How many of you are delivering new equipment to the patient if the patient wants a new capped rental? How is this regulated by medicare.

hi, i had my rul claim denied also, i had correct modifiers, rr ra, and added note:rul, initial date 121004. what exactly am i doing wrong, i canot get these to pay first time, i always have to send to review. thanks
