Default Diabetic supplies - Over utilization

How does everyone go about collecting documentation for Medicare patients that exceed the guidelines for utlilization? We are getting the information upon start of service, but do you collect this documentation every six months from the physician or patient? Does the physician have to sign off? Is a new RX/signature from the physician needed every 6 months for patients that over utilize?

Any help with this would be greatly appreciated!

Yes, definitely get copies of the testing log from the patient. Yes, you must get a new prescription for over-utilization patients every six months. You should also seek progress notes from the attending / treating physician about why the patient continues to need a higher testing frequency. That should be noted each time the patient visits the Physicians office. Be sure your patient is seeing the physician at least every six months.

where are you getting your info from? according to region B , we only have to get a new order for over utilization usage every 12 months.
