Manual Wheelchair Documentation
I need advice from someone who has been billing for manual wheelchairs for
some time and who has passed audits on their supporting documentation. I
have read all the LCD's thoroughly and I am very familiar with the coverage
criteria for the bases, accessories, and cushions. What I'm lost on is "how"
to obtain this documentation from physicians justifying the patient's need
for base and accessories that will pass audit?
For instance, how do you get the doctor to justify the need for accessories
such as:
. Lap belt
. Anti-Tip Safety Wheels
. Flip-up adjust height arm rests
. Brake handle extensions
Does the doctor have to write out a sentence in the actual patient's chart
justifying each accessory as is required for power mobility? I imagine it a
daunting task to get a doctor to write in the patient's chart "pt. requires
an extension mechanism in order to operate a breaking system because." for
each and every accessory? Or can the doctor just check off boxes on a
Detailed Written Order? Or can the provider prepare a statement of medical
necessity with the medical justifications for each accessory already stated
on the SMN and the doctor just signs? Can anyone provide me samples of the
detailed written order they use?
