Default Hospital Bed Extender Kits

We have a few patients that are too tall that require the extender kits... but I'm not really sure of the exact height requirement and if there is a specific code for the kit. We have been billing with E1399 and a narrative-description of the item. I believe I read in the article that all hospital bed accessories require the kx if coverage criteria had been met. I looked on Noridian and I cannot find anything that states anything about the extender kit. I need help and an answer!!!
