Default Billing for Auto Paps

If you charge an upgrade for an auto cpap or bipap for Medicare, how are you billing that? We know you need an ABN. If the upgrade charge is,say, $200, can we bill that to the patient upfront? Or do we prorate that out monthly on capped rental billing and bill each month as an upgrade?
