Anthem Billing
I have a patient that is a non Medicare patient. I am trying to bill the TENS unit we dispensed to him that was approved by his insurance Anthem. They tell me to paperbill the item and that I can use a CMS 1500. Does anyone know how to do that when they aren't a Medicare patient?
