Question Colistimethate - Medicare Part B or Part D? 2nd request for info

Good Morning:

Need verification for Colistimethate Sodium for inhalation. A Part D drug plan has denied auth stating it's covered under Part B? However, I don't see pricing for code under Part B? Can anyone advise?

also code?

150mg vial.. dilute and put into nebulizer

do we use: J7799 or S0142 for Inhalation?

DOES ANYONE HAVE ANY ANSWERS?

My sister is a pharmacist, and here is the answer she gave me-

Off-label uses:
Aerosolized colistin has been shown to be beneficial in the treatment of sensitive Pseudomonas infections in patients with cystic fibrosis. In addition, early aggressive use of the drug as adjunctive therapy may play a role in reducing chronic infections in this population.

This is what we see it requested for, not to be used IV or IM.

If it is administered via neb, it is Part B unless the pt is in LTC.

(since my post, I believe we did get the Part D plan to authorize)

?What code is used under Part B? Neither of the codes listed are showing as covered.
