Default documentation for pwc batteries
What documentation is needed to bill for a patient whom already has a pwc just needing batteries who has medicare

Doctors order and a Technician "eval" stating battery not in working order, delivery ticket and signed Assignment of Benefits (and anything else you would need in file with a delivery)
