Everything that I have read on the LCD for Oxygen only talks about Medicare HMOs to Medicare or Medicare secondary to Medicare primary. When a patient changes from Private (non medicare HMO) insurance to Medicare, is it necessary for new testing to be done and a new initial CMN to be completed in order to bill?

Yes, both new testing and a new initial CMN must be done.
