I'm being asked to supply one of these for a patient. I looked over the LCD and see how it is much more specific than normal K0001. Does anyone out there have any type of forms they have developed to help in getting all the info you need for this type of w/c prior to billing to ensure you are covered? any suggestions would be appreciated.

someone else that specializes in WC might be of more help. Our basic take is that K0005 are HAH - Hard As Hell - to get paid. We have very little luck and now use ABNs and collect cash and then file.
