hi, have a question not sure how to handle. i have old acct waiting for the face to face eval note to bill oxygen -was set up 041610, testing 040710. we could not locate a visit to doctor until 050310. so which is correct to bill? bill the 041610 date, attach cmn, with gz modifier, or bill may 16 2010 and attach cmn to this claim?
