Suction Catheters A4624 billing

Per the LCD and coverage Medicare states they will cover 3 a day for the patient. I am trying to find out how these are to be billed. I haven't billed them for a long time and can't seem to find whether you bill in advance for the Catheters or bill in the rears for the catheters? Any help would be appreciated.
