Has anyone ever had Medicare pay for oxygen with this answer on the CMN regarding testing? If so, what kind of documentation do you need?

You should not be getting paid for a number 3 on your CMN as far as we have been told (due to a denial) their system is set up to reject/deny claims as non-compliant if this is selected on the cmn.
