Default oxygen billing

I need some serious help
I have my first claim that I am billing for Oxygen , codes given to me are
E0431 Portable O2 ( new)
E1390 Oxygen Concentrator ( new )
I see on the CMN it states 86% and the highest level is 3
dx: is copd & lung cancer

Can someone tellme if there is a span date on the oxygen or do I just bill ..

Oxygen claims to Medicare do not require a span date for the service dates. They are billed with the same dates in the From/To fields.
