Does anyone know if it is a Medicare requirement to provide to the patient the cost of equipment? As in does it have to be on their delivery ticket the amount of each item billed to insurance and what they're portion is? I've worked in two different DMEs. One didn't put prices on the DT and the other one does.

I am looking for this exact information - are there published regulations by CMS that state what is required on the delivery ticket? I have found references to DT regulations but can't find them!
