Default Complex Sleep Apnea

What diagnosis code(s) is used for the pt initially diagnosed w/ OSA and then develops centrals on PAP. I'm unsure of what to use in the situation of Complex Sleep Apnea. Do you use Primary CSA (327.21) alone....or "CSA in conditions specified elsewhere" (327.27. I've read that if you use the 327.27 that it should be listed as a secondary dx. What would you put in the primary slot in that case??

If you are supplying RAD with Backup rate for treatment of the Central Apnea, that is the diagnosis you would use. Have plenty on documentation on hand regarding the central apneas. I think the LCD would specifically exclude the RAD with Backup if the diagnosis of OSA was listed.

I'm sorry.... can you clarify which code to use:
Primary CSA (327.21) alone....or
"CSA in conditions specified elsewhere" (327.27) with another dx code listed in the primary position

I would want to use 327.21, however you should confirm that dx code with the physician before proceeding.
