Based on personal experience, CPR+ from Definitive Homecare Solutions would be at the top of my list. They were leaps and bounds ahead of the other packages when we started looking 3 years ago, in terms of customization, inventory management, point of sale operations, and document management. We've been using them for a little over 2 years, and I'm constantly impressed with how often they release enhancements (for free), and how quickly they are able to adjust to industry changes and requirements. Definitely worth looking at.
