Default ENT Intake and Documention

Does anyone have a "policy" or "procedure" for taking on new Medicare Enteral patients. We are a fairly new DME company and have yet to establish this for product line. Also, when showing documentation for patients who have had there tube placement for years - how do we get the documentation that shows the need for the tube.
Thanks any help/assistance is much appreciated.

We require a swallow study or at least a speech eval on ALL ENT patients. We've been burned too many times not to. On a long term patient switching to our company we would still try to get it from either the hospital records or the existing DME company. Its too much money to risk and these days you know you'll be audited for it eventually!
