Medicare states that "Suppliers have the option of providing physicians with a form that lists the seven elements"

Handwritten forms have been rejected because they are 'illegible' and if the prescription is written on the new 'security' pads, they will not fax, scan nor copy. Has anyone had any sucess developing a process for the seven element requirement?

If it is the same 7 element order that is required for regular pmd's, read
the release at the link below. Simple to make your own but you should read
my post under the billing section about the pre payment audit that was
denied in part because our 7 element order said "Physician Order for Power
Mobility Device" and "List Dx relating to patients need for Power Mobility
Device" - That is considered "prompting" the doctor to order a pmd even
thought we are not telling them what to order. So be careful and leave it
generic - just like the sample below.

I just went to a Pride Mobility class and they have a form on their website that is perfectly legal to use. Go to Pride provider if you are a customer log in then go to Product Planning & Reimbusement. On the left you will see forms click on it and you will see the 7 element order there in the list.
