If the HMO is located near you, visit them in person to request being added to their network of providers.

Otherwise you will need to call or email their Ancillary Provider Network Manager to request being added to their network. They will need to know your service area and what you provide.

It also may help if you have some key physicians who want to use you, who can send a request to their Medical Director.

These days, many of the larger HMOs are limiting the number of providers, so it's not easy.
