Does anyone have any ideas on how to write up a care plan/outcome for a person receiving diabetic shoes at a pharmacy? We only provide the equipment and supplies and educate on those things, not disease process. I am a nurse and I cannot get my head wrapped around this one! This is required for our accredidation compliance. any suggestions would be helpful. Thanks!

We use a form called a plan of service (just like for other pieces of medical equipment).
